Physical Therapist - Home Health (Travel/Contract)
Location: Durham, North Carolina
Job Details
- Specialty: Therapy/Rehabilitation
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
- Hours per Week: 40 hours
- Average Patients per Day: 5-6
- Patient Visits: Both patient homes and care facilities
- Productivity Expectation: Approximately 30 units per week
- EMR System: Homecare Homebase
- Travel Requirements: Must be willing to travel up to 75 miles from home clinic; average travel about 300 miles per week between patient homes
- Schedule Variations: Most assignments are standard 5x8; some may have 4x10 shifts (Friday-Monday) depending on territory
Required Qualifications
- Minimum 1 full year of recent Adult Home Health experience (first-time home health therapists not accepted)
- Current Physical Therapist license in hand (pending licenses accepted only if license can be obtained within a few days)
- CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted)
- Reliable transportation and current driver's license
- Agency-required liability insurance
- Must be able to meet documentation and travel point system requirements (points assigned per visit, documentation, travel, etc.)
On-Call and Weekend Requirements
- On-call: Not required for this position
- Weekend work: Varies by location and must be discussed during interview
Additional Requirements and Compliance
- Must complete a variety of background checks including but not limited to:
- 7-year work and residence history verification
- Sex offender and nationwide criminal background checks
- State-specific registry and fingerprinting requirements (e.g., NC SBI fingerprinting mandatory, cannot start with results pending)
- Drug screening: 10-panel drug test required
- Health documents required prior to start and updated annually:
- Physical exam within 12 months confirming good health and no communicable diseases
- Negative TB test or chest X-ray within 12 months
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
- Seasonal flu vaccination or documented declination
- COVID-19 vaccination series or declination
- Must review and sign various compliance, confidentiality, and policy documents prior to start and annually thereafter
- Must have personal or auto insurance policy under candidate’s name or list candidate as a driver
Notes for Candidates
- This is a true contract assignment with possible hire at the end of the contract
- Local candidates are accepted at the same rate as travelers
- Mileage reimbursement follows IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable)
- Candidates must be prepared to participate in periodic office meetings and home visits
- Candidates should discuss any specific weekend or on-call requirements during the interview process
This position is ideal for experienced Physical Therapists with recent adult home health experience who are comfortable with travel and working independently in patient homes and care facilities.
