Description
Occupational Therapist School - Travel/Contract Position
Location: Medford, Oregon (worksite in Klamath Falls)
Contract Length: 36 weeks
Job Details
- Specialty: Therapy/Rehabilitation (Occupational Therapist)
- Work Schedule: Day shift, 5 days per week, 7.5 hours per day (8:00 AM - 5:00 PM)
- Caseload: Typically 12-15 students, can go up to 20
- Client Population: Students from Kindergarten through 12th grade, with a small caseload of early childhood
- Weekly Hours: 37.5 hours

Required Qualifications
- Valid Occupational Therapist license in Oregon
Additional Requirements
- Unexpired government-issued photo ID (Driver’s License or State ID)
- Required accounting and tax documents (W4, State-specific W4 if applicable, Permanent Tax Declaration, Direct Deposit Authorization and supporting documents)
- Emergency Contact Form and Equal Employment Opportunity (EEO) forms
- Arbitration Agreement (expires annually)
- Proof of name change if applicable
- FERPA agreement signed and updated annually (required for Educational Services candidates)
Optional / Client-Dependent Requirements
- Physical exam, TB testing, immunizations (Measles, Mumps, Rubella, Hepatitis B, Varicella), drug testing, flu vaccination only if specifically requested by the client or state
- Competency exams only if required by the school/client
- References and employment application only if requested by client
