Description
Physical Therapist (PT) - Long Term Care (LTC)
Location: Worchester, Massachusetts
Employment Type: Temporary
Contract Length: 13 weeks

Job Description
- Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care.
- Develop and implement appropriate plans of care following regulatory and clinical practice standards.
- Utilize standardized assessments and evidence-based practices to support clinical interventions.
- Administer rehabilitation procedures including manual techniques, ambulation, therapeutic exercises, modalities, use of assistive devices, and wound care.
- Assess patient progress and adjust treatments accordingly, documenting all in clinical records per regulatory requirements.
- Evaluate treatment outcomes and modify services based on patient evaluation; make referrals as needed.
- Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
- Communicate effectively with supervisors and healthcare team members about patient progress, barriers, and treatment plans.
- Prepare comprehensive discharge summaries in compliance with regulatory and clinical requirements.
- Document all required information accurately in electronic systems and maintain patient records per company policy.
- Record treatment minutes in accordance with regulatory and clinical standards.
- Participate in patient care conferences, utilization review meetings, rehabilitation meetings, and required trainings or in-services.
- Obtain necessary durable medical equipment (DME) or supplies as required.
- Supervise and support physical therapist assistants, rehabilitation aides, and/or students.
- Adhere to APTA and state-required practice acts, code of conduct, and ethics during professional practice.
Required Qualifications
- Minimum 1 year of Physical Therapist experience.
- New graduates accepted only if they have completed Level II clinical experience (not just Level I observation).
- Experience in Long Term Care (LTC) or Rehabilitation settings is required.
- Valid Massachusetts State Physical Therapist license.
Additional Information
- Workday consists of 8 hours.
- Overtime and holiday pay apply (not disclosed here).
