Description
Hospital Pharmacist (Day/Evening Rotating Weekends) - Colorado
Location
- Glenwood Springs, Colorado
Employment Type
- Contract
- Contract Length: 20 weeks
Schedule
- Rotating day/evening shifts, 40 hours per week
- Rotating weekends, 7:00 AM to 12:00 PM
- Occasional 3rd shift (overnight) as needed
Job Summary
- Prepare and dispense medication orders per physician requests
- Maintain medication stock in patient care areas
- Ensure safe, appropriate, and cost-effective medication therapies according to policies and protocols
Primary Duties and Responsibilities
- Interpret medication orders and maintain computerized patient medication profiles
- Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
- Comply with federal, state, and local laws; manage controlled substances and maintain required records
- Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and dose
- Perform pharmacokinetic drug dosing per hospital protocol
- Review and interpret culture and sensitivity data for antibiotic appropriateness and recommend changes
- Extract and interpret patient chart information accurately
- Detect and report adverse drug reactions
- Promote formulary adherence by minimizing non-formulary procurements and utilizing therapeutic substitution protocols
- Provide clinical consultation and clarification to practitioners, suggesting cost-effective therapeutic alternatives
- Provide drug information to patients and families as needed
- Document all clinical activities and interventions accurately
- Participate in quality improvement and medication review activities
- Collect data, conduct quality monitors and inspections, and maintain logs and records
- Assist in orientation, education, and training programs for pharmacy, nursing, and medical staff
- Serve as a member of the Code Response Team
Required Qualifications
- Valid Clinical Pharmacist license in Colorado
- Minimum of 3 years of clinical pharmacist experience, including experience within the last year
- BLS certification issued by the American Heart Association
- ACLS certification
- Experience with Epic electronic health record system

- Traveler’s permanent address must be at least 75 miles away from the facility (radius rule)
