Description

Physical Therapist (Outpatient) - Travel/Contract

Location: Cades, South Carolina
Contract Length: 13 weeks


Position Details

  • Shift: Days, Monday through Thursday 8:00 AM to 5:00 PM; Friday 8:00 AM to 12:00 PM
  • Hours per Week: 40 hours
  • Patient Ratio: 1:1 (45 minutes per session)
  • Weekend Requirement: None
  • On-call Requirement: No
  • Floating: Not required to float between units or facilities
  • Facility Type: Outpatient (not on hospital campus)
  • Charting System: Epic
  • Parking: Free

Required Qualifications

  • Valid Physical Therapist license in South Carolina
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in an outpatient setting
  • Basic Life Support (BLS) certification

Additional Requirements & Compliance

  • Background Checks:
    • Annual background check covering all locations lived/worked in the past 7 years
    • Includes SSN Trace, state and national criminal checks, sex offender registry, FACIS 3, and international searches
  • Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
  • Health Documents:
    • Physical exam updated annually, attesting ability to meet physical demands
    • Immunizations and titers required: Tetanus/Diphtheria/Tdap (within 10 years or declination), Measles, Mumps, Rubella, Hepatitis B (titer, vaccine series, or declination), Varicella
    • Negative TB test or chest X-ray within 90 days of start (2-step PPD or Quantiferon Gold; T-spot not permitted)
    • Annual TB questionnaire
    • Seasonal flu vaccination or medical/religious exemption required during flu season (Aug 1 - Mar 31)
    • COVID-19 vaccination highly preferred; if declined, must complete vaccination declination form
  • Respirator Clearance: Annual medical clearance required for respirator use via RespSafety virtual questionnaire
  • References: Proof of 2 clinical references or hospital evaluations
  • Performance Evaluation: Must be dated within 12 months of start
  • Employment Application: Complete application or resume required
  • Skills Checklist: Must be updated annually if applicable

Facility-Specific Requirements

  • Photo for Badge: Clear, passport-style photo with solid light background, frontal view only
  • Acknowledgments & Agreements: Must sign assignment acknowledgment, employee confidentiality agreement, HIPAA confidentiality agreement, code of conduct, systems user agreement, and complete user access request prior to start
  • Orientation: Completion of First Day Instructions (FDI) and OurDay modules prior to orientation; modules take approximately 2 hours and must be completed on a computer (not a phone)
  • Radiation Badge: Not required for this position
  • Local Radius: 60 miles
  • Locals: Not accepted

Important Notes

  • Facility may call off 1 shift per 2 weeks
  • Hurricane season (June through November) may require assignment to emergency response teams; avoid confirming new or extension assignments during peak months (August and September) if this is a concern
  • All licensure and certification documents must be valid beyond 30 days from start date; expiring credentials must be renewed or have a course scheduled before start
