PT Home Health - Travel/Contract Opportunity

Location: Saint Joseph, Missouri

Job Details

• Schedule:

  • 5 days per week, 8-hour shifts (8:00 AM – 4:30 PM)
  • Option for 4x10-hour shifts if preferred
• Call: Required 1 Sunday every 5 weeks
• Weekend Rotation: Every 5th week, work Saturday and Sunday doing admits, with 2 days off during the week
• Coverage Area: 13 counties in Missouri, with travel to more distant areas on a rotating basis; fewer patients scheduled on high mileage/travel days
• Dress Code: No printed scrubs; acceptable colors are black, grey, or khaki pants with polo or scrub tops

Required Qualifications

• Valid Physical Therapist license in Missouri
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Basic Life Support (BLS) certification

Compliance & Documentation Requirements

• Background checks within 30 days of start, including:

  • National criminal check, state and county searches (10 years, all aliases)
  • Social Security Number trace with aliases
  • National Sex Offender registry
  • Federal exclusion lists (EPLS/GSA/SAM, HHS/OIG)
• Education verification with official transcript or diploma including graduation date
• Missouri EDL Search within 30 days of start
• Drug screen within 30 days of start (includes multiple drug panels)
• Physical exam within last 12 months (not required annually)
• Fit mask testing completed onsite at orientation
• Immunizations/vaccinations or documented immunity/declination for:

  • Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer)
  • Hepatitis B (vaccination series, titer, or declination)
  • Varicella (2 vaccines, positive titer, or declination)
  • Seasonal Flu vaccine required during flu season or approved medical/religious exemption (mask required if exempt)
• Negative TB test or chest x-ray within 1 year; additional requirements if positive TB history
• Completion of facility-specific confidentiality agreement, code of conduct, demographic form, and orientation quiz (minimum 84% score)

• Two professional references within 3 years of start date
• Employment application and skills checklist required
• Verification of most recent employer required
• Scrubs must be plain (no prints) and in approved colors
• Locals accepted at the same rate as travelers
