Physical Therapist - Home Health (Travel/Contract)

Location: Garden City, New York

Job Details

• Shift: Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
• Average Patients per Day: 5-6
• Weekly Productivity Expectation: 30 points per week (points assigned based on visits and activities)
• Typical Schedule: Mostly standard 5x8; some positions may have 4x10 shifts (Friday-Monday)
• Weekend Rotation: Yes, approximately every 4 weeks (Saturday only)
• On-Call: No on-call required
• Travel Requirements: Must be willing to travel up to 75 miles from home clinic; expected weekly travel 250-300 miles
• Work Setting: Home Health (home visits with periodic office meetings)
• EMR System: PointCare

Required Qualifications

• Valid Physical Therapist license in New York
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• BLS certification
• Reliable transportation and current driver's license
• Agency-required liability insurance
• Must be able to meet documentation and travel requirements for home health visits

Additional Requirements & Compliance

• Must complete and maintain all required background checks, including but not limited to:

  • 7-year work/living history background check
  • Sex offender and state-specific registry searches as applicable
  • Fingerprinting as required by state regulations
• Required health documentation includes:

  • Physical exam within 12 months prior to start
  • Negative TB test or chest X-ray within 12 months
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
  • Drug screening (10-panel)
  • Seasonal flu vaccination or documented declination
  • COVID-19 vaccination series or declination
• Must complete agency and facility-specific compliance training and policy acknowledgements prior to start, including:

  • HIPAA and privacy policies
  • Code of conduct and confidentiality agreements
  • Home Health Core Care Policy and scenarios
  • COVID-19 policy and attestation
• Two professional references required
• Verification of at least two most recent employers or last 7 years of work history

Notes

• No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours
• Local candidates accepted at the same rate
• Weekend and on-call requirements may vary by location; discuss during interview
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed)
