Description

Registered Nurse (RN) - Long Term Care (LTC)

Location: Mt. Pleasant, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: Evenings, 2:00 PM – 10:30 PM (8-hour shifts)
Schedule: 40 hours one week, 32 hours the following week (72 hours total in 2 weeks)
Weekend Requirement: Every other weekend required


Job Description

  • Provide routine daily nursing care and other services based on resident assessments, choices, and care plans.
  • Assigned to a primary household of approximately 16 residents.
  • Collaborate closely with RN Manager to manage day-to-day functions.
  • Provide skilled nursing care for residents with complex needs.
  • Candidate responsible for own transportation; public transportation is extremely limited.
  • Previous EPIC experience required.

Required Qualifications

  • Valid RN license in Iowa.
  • Associate of Science in Nursing / Associate Degree in Nursing.
  • Minimum of 2 years of Long Term Care experience, including experience within the last year.
  • ACLS certification.
  • Experience with Epic electronic health record system.

Onboarding and Compliance Requirements

  • Primary source verification of RN license within 30 days of start.
  • Education verification through third party; diploma/transcripts may be required.
  • Drug screen (9-panel, excludes THC) within 30 days of start.
  • Skills checklist completed within the last 12 months.
  • COVID-19 vaccine accepted with declination option.
  • MMR: 2 vaccines or positive titer; 1 booster or declination accepted if low/equivocal.
  • Fit test for approved respirators completed prior to start.
  • Hepatitis B: 2 vaccines or positive titer; 1 booster or declination accepted if low/equivocal.
  • TB test: Negative test within past year (TSPOT or QuantiFERON IGG) or documentation if history of positive test.
  • Health statement from physician within last 12 months.
  • Current driver’s license or state ID valid through assignment.
  • Varicella: 2 vaccines or positive titer; 1 booster or declination accepted if low/equivocal.
  • Tdap valid within 10 years; declination accepted.
  • Background check within 30 days of start including SS trace and 7-year county search.
  • Iowa Adult and Child Abuse clearance (state requirement).
  • EPLS/GSA/SAM HHS/OIG check within 30 days of start.
  • Two professional references required; one must be from a supervisor.
  • Resume or profile submission required.
  • I-9 attestation on agency letterhead.
  • Color vision verification.
  • Contract employee data sheet.
  • National Sex Offender Registry (NSO) check within 30 days.
  • OFAC check within 30 days.
  • OSHA questionnaire completed by candidate.
  • Epic registration acknowledgment form signed and uploaded after registration.

Additional Notes

  • Influenza vaccine is optional; facility declination accepted (seasonal requirement Sept 1 – March 31, due by Dec 1 annually).
  • Candidate must be prepared for limited public transportation options and provide own transportation.

This position is ideal for RNs with experience or interest in long-term care nursing, comfortable working evening shifts, and able to manage a primary group of residents with complex needs.
