PT Home Health - Oregon (Travel/Contract)

Job Details
- Employment Type: Travel/Contract
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Contract Length: 13 weeks
- Patient Ratio: 1:1, may visit up to 6 patients per day
- Productivity Standard: 30 productivity points per day
- Weekend Work: Not required
- On-Call: Not required
- Floating Between Units: Not required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have own transportation; daily travel radius up to 60 miles (60-90 minutes total drive time)
- Attire: Business casual; scrubs allowed but not required; no jeans
- Charting System: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation required
- Parking: Free
- Mileage Reimbursement: IRS standard
Required Qualifications
- Valid Physical Therapist license in Oregon
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
Compliance and Documentation Requirements
- Background Check: Must be completed within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide sex offender check, motor vehicle record, and state-specific abuse/neglect searches as applicable
- Drug Screen: 9-panel drug screen within 12 months of start; negative results required
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate ability to work with or without accommodation
- Immunizations and Health Requirements:
- Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months (annual updates required)
- Measles, Mumps, Rubella: 2 MMR vaccines or positive titer (declination accepted)
- Hepatitis B: Documentation of vaccination series or signed declination
- Varicella: 2 vaccines or positive titer (declination accepted)
- Seasonal Flu Vaccine during flu season (declination accepted)
- COVID-19 vaccination record preferred; declinations allowed with adherence to masking policies
- Respirator Clearance: Required annually for travel candidates; virtual questionnaire reviewed by licensed healthcare professional
- Fit Mask Testing: Required annually; completed onsite if no valid record within 1 year
- References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application: Must include at least last 5 years of employment with month/year format; gaps and overlaps must be explained
