Job Opportunity: Home Health Registered Nurse

Location: Portland, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
  • Patient Ratio: 1:1, with up to 6 patient visits per day (30 productivity points standard)
  • Start Date: ASAP
  • Schedule Notes:
    • No 4x10 or 3x12 shifts available
    • No weekend requirement
    • No on-call duties
    • No floating between units
    • Block scheduling not accommodated
  • Local Candidates: Accepted within a 50-75 mile radius, considered local at the same rate
  • Transportation: Must have own transportation; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
  • COVID-19 Vaccination: Primary series plus one booster required

Required Qualifications

  • Valid Registered Nurse (RN) license in Oregon
  • Minimum 1 year of experience in Home Health nursing, with experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association or Heart and Stroke Foundation of Canada
  • Neonatal Resuscitation Program (NRP) certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Experience with OASIS documentation in a Home Health setting

Facility & Work Environment

  • Type of Facility: Home Health
  • Attire: Business casual; scrubs allowed but not required; no jeans
  • Charting Systems: EPIC Remote Client (laptop), Rover (phone app), and OASIS
  • Parking: Free
  • Mileage Reimbursement: IRS standard rates

Health & Safety Requirements

  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness for duty
  • Immunizations & Testing:
    • COVID-19 vaccination (primary series + booster)
    • Flu vaccine during flu season or signed declination
    • Tetanus/Diphtheria/Tdap within 10 years or declination
    • Measles, Mumps, Rubella (2 MMR vaccines or positive titer or declination)
    • Varicella (2 vaccines or positive titer or declination)
    • Hepatitis B vaccination series or signed declination
    • Negative TB test or chest X-ray within 12 months, updated annually
  • Drug Screening: 9-panel drug screen within 12 months of start
  • Respiratory Safety Clearance: Required annually for respirator use (travel only)
  • Fit Mask Testing: Required annually for travel candidates; may be completed onsite

Background & Compliance

  • Background Check: Must be completed within 90 days of start, covering all locations lived/worked in past 7 years, including SSN trace, death master index, sex offender registry, motor vehicle record, and state-specific abuse/neglect checks as applicable
  • Education Verification: Required for nursing degree; international education requires diploma/transcripts or CES report
  • State-Specific Checks: Required for Oregon and Washington placements, including WATCH and OBRA registry searches as applicable
  • Employment Application: Must include at least last 5 years of employment with month/year format for dates worked

Additional Notes

  • Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks)
  • Float Policy: No floating required for this position
  • Annual Training Modules: Not required for Home Health at this facility
  • Photo for Badge: Passport-style photo with neutral background required prior to start
  • References & Work History Verification: Required to support credentialing and compliance

This position is suitable for first-time travel nurses and local candidates within the specified radius. Candidates must be prepared for travel and have reliable transportation due to the geographic spread of patient visits.
