Required Qualifications

• Minimum of 1 year of experience as a Surgical Technologist
• Recent Surgical Technologist experience within the last year
• Experience as an OB Technician

• Weekend work: Required every other weekend
• Block scheduling: Required

Background & Compliance Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Social Security Card (as applicable for I9 verification)
• Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
• Education verification required for all candidates
• State-specific searches as applicable (Oregon, Washington)
• References: Minimum 2 references covering 1 year of employment in a comparable specialty/setting within last 3 years
• Employment application covering at least last 5 years of employment
• Skills checklist updated annually as applicable
• Work history verification required if exceptions to references are requested

Health & Safety Requirements

• Physical exam within 1 year of start, signed by MD, NP, PA, or DO
• Fit mask test (respirator clearance) required for travel candidates, updated annually
• Immunizations and documentation:

  • Tetanus/Diphtheria/Tdap (within 10 years; declination accepted)
  • Negative TB test or chest X-ray within 12 months, updated annually
  • Measles (Rubeola), Mumps, Rubella: 2 MMR vaccines or positive titer; declination accepted
  • Hepatitis B: Vaccination series or signed declination; positive titer documentation accepted
  • Varicella: 2 vaccines or positive titer; declination accepted
  • Seasonal Flu vaccine during flu season (declination accepted)
  • COVID vaccination record preferred; declinations not required but unvaccinated must follow infection prevention protocols
• Drug screening: 9-panel drug screen within 12 months of start; updated as needed

Training & Onboarding

• Completion of annual regulatory and specialty-specific training modules via LMS (Workramp)
• Epic system training modules required prior to or shortly after orientation
• Facility-specific float policy acknowledgment required (may float to other nearby facilities)

• Badge photo required (passport style with neutral background)
• No 48-hour shifts approved
• No parking fees for staff
• Facility uses Epic for charting
