Description
Physical Therapist - Home Health (Travel/Contract)
Location: North Charleston, South Carolina
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40
Job Description
- Provide physical therapy services in patients' homes and occasionally in care facilities.
- Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points minimum per week).
- Types of cases include Start of Care and Follow Ups.
- Must have reliable transportation and a current driver’s license.
- Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
- Willingness to travel up to 75 miles from the home clinic.
- Schedule may vary; some positions may have a 4x10 shift Friday-Monday, but most are standard 5x8 Monday-Friday.
- Potential for contract extension or hire based on performance.
Required Qualifications
- Valid Physical Therapist license in South Carolina.
- Minimum of 1 year of Physical Therapist experience within the last year.
- Experience in a Home Health setting.
- Current BLS certification.
Additional Requirements & Compliance
- Must complete a standard 10-panel drug screen.
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases; updated annually.
- Negative TB test or chest X-ray within 12 months, updated annually; results cannot be pending at start.
- Immunization or titer proof required for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination required.
- Background checks including 7-year history, sex offender search, and state-specific screenings as applicable.
- Fingerprinting required if not residing in South Carolina for 12 months prior to start.
- Verification of education and work history (at least two most recent employers or last 7 years).
- Two professional references required.
- Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually if working more than one year consecutively.
- Personal or auto insurance policy required for home health placements, must be current and list candidate as insured or driver.
Work Environment & Expectations
- Visits include both patient homes and care facilities.
- No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.
- On-call requirements vary by location and should be discussed during the interview.
- Weekend work requirements vary and should be clarified during the interview.
- EMR systems used include HomeCare HomeBase, Oasis, and Outlook.
Travel & Local Candidates
- Local candidates accepted at the same rate.
- Must be willing to travel up to 75 miles from the home clinic location.
- Average travel expected: approximately 30-40 miles per day, visiting 5-6 patients daily.
