Home Health Registered Nurse (RN) – Vermont

Job Description

Provide a full range of professional nursing services to clients of all ages in a home health and hospice setting
Responsibilities include admission, evaluation, and care plan development
Collaborate with a team of homecare professionals and paraprofessionals to deliver coordinated care

Required Qualifications

Valid RN license in Vermont
Minimum 1 year of home health nursing experience within the last year
BLS certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Additional Information

Work area includes Caledonia and southern Essex counties
