Shift Details
- Night shift
- 3 shifts per week, 12 hours each (18:30 to 7:00)
- Every other weekend rotation required upon request
Facility Type
- Skilled Nursing and Rehabilitation Units within a Long-Term Care (LTC) facility
- Acts similarly to a Med-Surg environment
- Average daily census: 72 residents
- Direct patient care ratio: up to 20 residents per nurse
Job Description
- Provide nursing care in skilled nursing and rehab units
- Rehab experience with strong Med-Surg and acute care background is helpful but not required
- Dementia training or experience is highly recommended
- Work closely with CNAs and HCAs
- Use of special lifting equipment required
- Provide specialized medical and rehabilitation care to dementia and surgical patients, including IV medications and wound care
Required Qualifications
- Valid Registered Nurse (RN) license in Illinois
- Minimum 2 years of skilled nursing experience, including experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- BLS (Basic Life Support) certification
- ACLS (Advanced Cardiovascular Life Support) certification
- IV Certification
Scheduling & Time Off
- Every other weekend rotation required upon center request
- Cannot guarantee all holidays off
- Any call-outs must be given with more than 24 hours' notice

- Any shade of blue scrubs

Background & Compliance Requirements
- Background checks required within 1 year of start and updated annually, including:
- Social Security trace
- County searches for last 7 years of residence/work/school
- Nationwide sex offender search
- EPLS/GSA/SAM and HHS/OIG checks
- Professional license primary source verification required
- Unit-specific competency exam and medication/pharmacology competency exam required
- Physical exam required within 1 year of start, updated annually (can be completed by NP or PCP)
- Immunization and health document requirements include:
- BLS, ACLS certifications
- Tetanus/Diphtheria/Tdap vaccination or declination (within past 10 years)
- Negative TB test or chest X-ray per protocol
- Measles, Mumps, Rubella (MMR) immunity or vaccination/declination
- Hepatitis B declination form
- Varicella immunity or vaccination/declination
- Seasonal flu vaccination or medical/religious exemption
- COVID-19 vaccination required prior to start (medical/religious exemptions accepted with testing requirements)
- 9-panel drug screen within last 12 months (no pending drug screens allowed)
- Two professional references required within 2 years of start
- Employment application and unit-specific skills checklist required within 1 year of start
Additional Notes
- Rehab experience with dementia care is highly recommended
- Strong Med-Surg and acute care experience beneficial
- Must be able to work with lifting equipment
- Excellent communication and teamwork skills essential
