Description
Physical Therapist (Outpatient) - Travel/Contract
Location: Perry, Oklahoma
Contract Length: 13 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 40 hours/week)
Job Summary
- Provide Physical Therapy treatments independently for patients with disease or disability.
- Deliver preventive health measures.
- Provide technical direction to students, therapy assistants, and technicians.
- Work setting is a small hospital environment.
Required Qualifications
- Valid Physical Therapist license in Oklahoma.
- Minimum 1 year of Physical Therapist experience within the last year.
- Graduate of a school of physical therapy with a Bachelor of Science degree.
- Basic Life Support (BLS) certification.
Compliance and Documentation Requirements
- Government-issued photo ID required.
- E-Verify or attestation required.
- Background checks including 7-year criminal history, sex offender registry, and federal exclusions must be completed prior to start.
- Primary source verification of state license required.
- Professional references: 2 required within 3 years of start.
- Employment application and annual skills checklist required.
Certifications and Health Requirements
- BLS certification (AHA or ARC) mandatory.
- Tdap vaccine within 10 years or declination accepted.
- Negative TB test or chest X-ray within 12 months.
- Required immunizations or positive titers for: Measles, Mumps, Rubella, Varicella (declinations/exemptions not accepted for these).
- Hepatitis B vaccination series, positive titer, or declination required.
- Seasonal flu vaccination or approved declination required during flu season (Oct 1 – Mar 31).
- COVID vaccination or approved declination form required.
- 10-panel drug screen within 1 year prior to start (amphetamines, barbiturates, benzodiazepines, cocaine, opiates, oxycodone, marijuana, methadone, fentanyl).
- Physical exam within 1 year of start.
Orientation and Facility Requirements
- Completion of facility-specific orientation modules (2-3 hours) prior to start, including safety and confidentiality training.
- Review and sign facility commitment to excellence standards and confidentiality agreements before starting.
- Passport-style photo required for badge.
- Fit testing and color-blind exam will be completed onsite during orientation.
Additional Notes
- Candidate will work independently in a small hospital setting.
- Prior travel experience is not explicitly required.
- No advanced cardiac life support (ACLS), pediatric advanced life support (PALS), or neonatal resuscitation (NRP) certifications required for this role.
