Physical Therapist Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 08:00 AM – 04:30 PM
Job Summary
- Provide physical therapy services to patients in their homes who have physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Patient age range: birth to over 100 years.
- Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services.
- Maintain thorough and timely electronic documentation and interdisciplinary communication according to agency policies and standards of practice.
- Weekend rotations occur once every 6 weeks.
- Ability to travel to assigned cases is required.
Required Qualifications
- Valid Physical Therapist license in Massachusetts.
- Minimum 1 year of experience as a Physical Therapist.
- Physical Therapist experience within the last year.
- Experience in a Home Care setting.
- Minimum 1 year of case management experience.
- Must be able to travel to patient locations as assigned.
Onboarding and Compliance Requirements
- Submission of a recent (within 6 months) professional color photo (no selfies, plain background, no eyeglasses).
- Motor Vehicle Record (MVR) check within 45 days of start.
- Primary source verification of professional license/certification within 30 days of start.
- N95 Respirator Fit Test within 365 days of start, updated annually (no exemptions allowed).
- Nurse Aide Registry check within 30 days of start (required for all employees).
- Respirator Medical Clearance attestation if fit test is required.
- Documentation of immunity or vaccination for Measles, Mumps, and Rubella (MMR).
- Tuberculosis (TB) test documentation within 3 months of start, updated if break >6 months.
- Physician-signed health statement within 365 days of start, updated if break >6 months.
- Criminal background check within 45 days prior to start, updated if break >6 months.
- Massachusetts CORI criminal history check within 30 days prior to start, updated if break >6 months.
- Education verification of highest level achieved.
- Social Security Number trace as part of criminal background check.
- National sex offender registry check within 30 days prior to start, updated annually.
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start, updated annually.
- Completion of skills checklist within 1 year of submission.
- Submission of resume including past 7 years of work experience with city and state.
- Two professional references within 4 years, including at least one supervisor or higher.
- Confirmation of Assignment/POI form required for all initial assignments and extensions.
- Candidate submission coversheet required for initial assignment submissions.
Additional Notes
- Positive TB skin test requires documentation of a negative chest x-ray.
- Name change documentation required if credential names do not match legal name.
- No religious or medical exemptions allowed for fit testing.
- No onsite fit testing available; must be completed prior to start date.
