Physical Therapist - Home Health (Travel/Contract)

Location: West Plains, Missouri
Contract Length: 13 weeks

Job Details

• Specialty: Therapy/Rehabilitation
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours)
• No weekends, call, or floating required
• Caseload: 8-11 patients daily depending on travel time
• Productivity expectation: 4-5 visits per day depending on area and travel
• Travel radius: 60 miles from home base

Required Qualifications

• Valid Physical Therapist license in Missouri
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification issued by American Red Cross or American Heart Association

Documentation & Compliance Requirements

• Background checks: 7-year criminal history, instant background acceptable, EPLS/GSA/SAM, HHS/OIG checks required and updated per contract renewal
• Physical exam: Initial and annual attestation of ability to meet physical demands
• Immunizations and health screenings:

  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella (MMR) immunity via titer or vaccination
  • Hepatitis B immunity or declination
  • Varicella immunity or declination
  • Seasonal flu vaccine or declination (required Oct 1 - Mar 31)
  • COVID-19 seasonal vaccine or approved medical/religious exemption
• Drug testing: 9-panel drug screen within last 12 months (instant acceptable)
• Proof of 2 clinical references or hospital evaluations
• Completion of employment application/resume
• Annual skills checklist and competency exams as applicable
• Facility-specific policies include nicotine-free campus and dress code adherence

• Electronic Medical Record (EMR): Meditech

Additional Notes

• Mileage reimbursement at IRS standard rate
• First-time travelers accepted
• One other therapist on staff at this location
