Work Schedule

  • Day shift, Monday through Friday
  • 8 hours per day (8:00 AM – 5:00 PM)
  • Weekend work may be required depending on census needs but schedule will be adjusted to maintain a 40-hour workweek
  • On-call may be needed occasionally but not regularly

Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses
  • Manage an average caseload of 5-8 patients per day, including start of care, wound care, and follow-up visits
  • Must be willing to travel up to 60-75 miles radius from home clinic, averaging approximately 200 miles traveled per week
  • Documentation and charting using HomeCare HomeBase (HCHB) and OASIS systems; proficiency required within the last 6 months or retraining provided

Required Qualifications

  • Valid RN license in Oregon
  • Minimum 1 year of hospice experience within the last year
  • Experience in a Home Health setting
  • BLS certification
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Proficiency with HomeCare HomeBase documentation system

Additional Information

  • First-time travelers are not accepted; recent specialty experience is mandatory
  • Local candidates accepted within the same pay structure
  • Dress code: Scrubs
  • Productivity expectations: 25-30 units per week including admissions
  • The position is due to increased census from a hospice closure in the area

Notes

  • Mileage reimbursement follows IRS standard rate for travel between patient homes; commute to/from first and last patient is not billable
  • Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 schedules
  • Weekend and on-call requirements vary and should be discussed during the interview process
