Description
Job Opportunity: Pharmacist Inpatient
Location: Lebanon, Pennsylvania
Employment Type: Travel/Contract
Contract Length: 13 weeks
Schedule & Shift Details
- 40 hours per week
- Shift: Day shift, 6:00 AM to 2:30 PM (5 days x 8 hours)
- Weekend Requirement: Every 3rd or 4th Saturday, 8:00 AM to 4:30 PM
- Holidays: As needed
- No call or floating required
Job Description
- Work in a Pre-Screen Unit; clinical fit will be assessed before final manager interview
- May care for COVID-19 patients
- Facility is tobacco and fragrance free
Required Qualifications
- Valid Hospital Pharmacist license in Pennsylvania
- Minimum 2 years of hospital pharmacist experience, including experience within the last year
- Experience with Epic system
Compliance & Documentation Requirements
Background Checks & Verifications
- 7-year criminal background check required within 30 days of start, updated annually
- Pennsylvania State Police PATCH search required, must be finalized prior to start (no pending results)
- PA FBI fingerprinting required within 1 year of start; candidate may start with results pending but must have results on file within 45 days or will be removed from assignment
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
- Education verification of highest degree required
- 2 professional references within 3 years required
- Most recent resume or employment application required
- Annual skills checklist required
Certifications (as applicable)
- Basic Life Support (BLS) - AHA or ARC provider card required
- Advanced Cardiac Life Support (ACLS) - AHA or ARC provider card required
- Other certifications (PALS, NRP, TNCC, NIH Stroke, etc.) as applicable per unit/job description
Competency Exams
- Medication Test/Pharmacology Competency Exam required as applicable
- Dysrhythmia/EKG Competency Exam required for all RNs (not applicable here but noted for awareness)
Facility-Specific Documents
- Clear, passport-style photo for badge
- Review and sign facility-required documents including Code of Conduct, Confidentiality Agreement, Commitment to Health & Wellness, Non-Employee Affidavit, and Hepatitis B declination form if applicable
- Complete Learning Management System (LMS) modules on PC/laptop prior to start (for RNs and CNAs; verify if applicable)
- New Hire Orientation must be completed by end of first day
Health & Immunization Requirements
- Physical exam within 90 days of start, signed by MD, DO, PA, or NPT
- Fit mask test required 3 business days prior to start for patient-facing roles
- Tetanus/Diphtheria/Tdap vaccine within 10 years of start
- Negative TB test (QGold or TSpot) within 90 days of start; PPD not accepted
- Vaccination or positive titer proof required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (positive titer or full vaccine series)
- Varicella
- Seasonal flu vaccine or valid medical/religious exemption required during flu season
- COVID-19 vaccine or valid medical/religious exemption or declination accepted
- Drug testing: Negative 9-panel required within 1 year of start (Aya requirement only)
- Respiratory safety clearance required within 1 year of start, renewed annually
Additional Notes
- Return to Office (RTO) requirement: maximum 7 days
- Tobacco and fragrance free environment; candidates must be notified prior to submission
- Candidate must provide government-issued photo ID
- Attestation forms and I9/E-Verify documentation required by agency
- All health documents must include two identifying markers (e.g., full name, DOB, SSN)
This position is ideal for a licensed pharmacist with inpatient experience, comfortable working day shifts with occasional weekend coverage, and able to meet comprehensive compliance and health requirements.
