Job Opportunity: Home Health Registered Nurse

Location: Lebanon, Pennsylvania (serving Adams and Franklin Counties)
Employment Type: Travel/Contract
Contract Length: 13 weeks


Schedule & Work Details

  • 40 hours per week, Monday to Friday, 8:00 AM to 4:30 PM
  • Weekend rotation: every 4th weekend
  • Floating between counties possible if patient care requires
  • Holidays as needed
  • No call requirements
  • May care for COVID-19 patients
  • Tobacco and fragrance-free facility

Required Qualifications

  • Valid RN license in Pennsylvania
  • Minimum 2 years of Home Health nursing experience, including experience within the last year
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Current Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross

Compliance & Documentation Requirements

  • Background checks including:
    • 7-year criminal background check (must be finalized prior to start)
    • Pennsylvania State Police PATCH search (must be finalized prior to start)
    • EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
  • Pennsylvania FBI fingerprinting required within 1 year of start; candidate may start with results pending but must have results on file within 45 days or will be removed from assignment
  • Pennsylvania Child Abuse (Childline) search required 3 business days prior to start; cannot start with results pending
  • Pennsylvania Department of Aging fingerprint clearance required for non-PA residents
  • Education verification of highest degree (degree, diploma, or transcripts)
  • Negative 9-panel drug test within 1 year of start (specific to agency requirements)
  • Two professional references within 3 years
  • Most recent resume or employment application
  • Annual skills checklist

Health & Immunization Requirements

  • Physical exam within 90 days of start, signed by MD, DO, PA, or NPT
  • Fit mask test required 3 business days prior to start for new or expiring fit tests
  • Tetanus/Diphtheria/Tdap vaccine within 10 years of start
  • Negative QGold or TSpot TB test within 90 days of start (PPD not accepted)
  • Measles (Rubeola), Mumps, Rubella, Varicella: 2 vaccines or positive titer required; boosters required if titers are negative or equivocal
  • Hepatitis B: positive titer or full vaccine series (3 doses) or declination form required
  • Seasonal flu vaccine or valid medical/religious exemption required during flu season
  • COVID-19 vaccine, medical/religious exemption, or declination accepted
  • Respiratory safety clearance within 1 year of start, renewed annually

Certifications & Competencies

  • Acknowledgement of Pass/Fail Testing required within 48 hours of signed contract (except OR and Home Health)
  • Medication/Pharmacology competency exam as applicable
  • Dysrhythmia/EKG competency exam required for all RNs except OR and Home Health; must be completed within 72 hours of contract signing
  • ACLS, PALS, NRP, TNCC, NIH Stroke Certification as required by unit/job description

Facility-Specific Requirements

  • Clear, passport-style photo for badge (professional appearance, light background, no face coverings except religious headwear)
  • Copy of auto insurance required for candidates working in home care
  • Completion and signing of facility-specific documents including:
    • Medical Statement Non-Employee Ability to Work form
    • Commitment to Health & Wellness
    • Code of Conduct
    • Confidentiality Agreement
    • Non-Employee Affidavit
    • I9 Attestation and Documentation Attestation (completed by agency)
    • Residency form for home care candidates
    • Hepatitis B declination form if applicable
    • Safety and Security Vehicle Registration (for home care)
    • Review of facility policies such as "Wellspan Way" description
  • Completion of Learning Management System (LMS) modules on PC/laptop prior to start (not accessible via cell phone)
  • New hire orientation must be completed by end of first day

Additional Notes

  • Return to Office (RTO) maximum 7 days
  • Candidate must notify if unable to comply with tobacco/fragrance-free policy
  • Clinical fit will be determined by clinical screening and manager interview on a case-by-case basis
