PT Home Health - Travel/Contract
Location: Seaside, Oregon
Contract Length: 13 weeks
Job Details

- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Typical Patient Load: 1:1 ratio, up to 6 patients per day
- Productivity Standard: 30 productivity points per day

- Locals Accepted: Yes, within 50-75 mile radius

- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Transportation: Traveler must have their own transportation
- Drive Time: Occasional max radius ~60 miles (60-90 minutes total drive time per day)
- COVID Vaccination: Primary series plus one booster required
- Documentation/Charting: EPIC Remote Client (laptop), Rover app (phone), and OASIS required
Required Qualifications
- Valid Physical Therapist license in Oregon (pending licenses accepted)
- Minimum 1 year of Physical Therapist experience, including experience within the last year
- Experience in a Home Health setting
- BLS (Basic Life Support) certification required

- Facility attire: Business casual; scrubs allowed but no specific color required; no jeans
- Parking: Free
- Mileage reimbursement: IRS standard
- Background check: Must be within 90 days of start, covering all locations lived/worked in past 7 years, including SSN trace, death master index, sex offender registry, motor vehicle record, and state-specific abuse/neglect searches as applicable
- Education verification required for all candidates
- Physical exam and health documents must be updated annually
- Fit mask testing required annually (onsite if no valid record within 1 year)
- Must complete required facility-specific and regional training modules as applicable (not required for home health at this facility)
- Traveler may be required to complete float policy acknowledgement (though floating is not currently required for this position)

Summary
This is a travel Physical Therapist position in home health based in Seaside, Oregon, requiring at least 1 year of skilled home health PT experience.
