Physical Therapist - Outpatient

Location: Pleasant Prairie, Wisconsin
Employment Type: Travel/Contract

Job Details

• Start Date: July 13
• Shift: Day shift, 4 days per week, 10 hours per day (7:00 AM - 5:00 PM)
• Setting: Outpatient clinic
• Experience Required: Minimum 1 year of outpatient physical therapy experience with neuro experience
• First Time Travelers Accepted: Yes
• Locals Accepted: Yes (at a lower rate)
• Pending License Accepted: Yes

Required Certifications

• Basic Life Support (BLS) (ARC or AHA acceptable; Provider, Instructor, and RQI accepted)

Required Qualifications

• Valid Physical Therapist license for Wisconsin
• Outpatient and neurological therapy experience (minimum 1 year)

Background and Compliance Requirements

• 7-year background check covering all locations lived, worked, or attended school within the last 7 years
• State-specific caregiver screening for Wisconsin
• Photo for badge (passport-style, shoulders up, no filters)
• Confidentiality statement signed prior to start
• Compliance checklist completed by agency representative

Health and Safety Requirements

• Physical exam within 1 year of start date
• Negative QGold TB test within 1 year (TSpot/PPD not accepted) or negative chest X-ray within 60 days of start
• Immunization or positive titer proof for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (titer, series, or declination)
• Negative 10-panel drug screen with expanded opiates within 14 days before start (instant tests not accepted)
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March)

Additional Requirements

• Two professional references verified within 2 years of start
• Annual skills checklist and competency exams as applicable
• Medication/pharmacology competency exam as applicable to profession

Notes

• Prior travel experience is not required
• ACLS, PALS, NRP, NIH Stroke Certification, and other certifications are not required for this position
• Flu vaccine exemptions require completion of facility-specific exemption forms at orientation if applicable
