Pharmacist Outpatient - Travel/Contract Position

Location: Kingman, Arizona
Contract Length: 13 weeks

Job Details

• Employment Type: Travel/Contract, full-time
• Shift: Variable between 8:00 AM to 8:00 PM (Day shift typically 8:00 AM to 4:00 PM, 5 days a week, 40 hours/week)
• Weekend work required
• On-call: Not required
• Flexible scheduling possible, but preference for minimal schedule restrictions
• First-time travelers accepted

Required Qualifications

• Minimum 5 years of ambulatory pharmacist experience, including experience within the last year
• Graduate of a Doctor of Pharmacy (PharmD) program
• Basic Life Support (BLS) certification required
• Pharmacist experience

Compliance and Documentation Requirements

• Background checks including 7-year search within 30 days of start, updated annually (SS trace, county searches, nationwide, sex offender, education verification)
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Transcript/Diploma and education verification through primary source
• Medication/Pharmacology competency exam as applicable
• Physical exam within 1 year of start
• Negative TB test or chest X-ray within 1 year, with annual TB questionnaire
• Immunization or titer proof for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 30 days of start (instant drug screen acceptable)
• Seasonal flu vaccination or declination during flu season (Oct 1 - Mar 31)
• Two professional references within 3 years
• Annual skills checklist

• Clinician interview required
• Locals and first-time travelers are compensated equally
• Facility-specific forms and agreements must be completed as a priority
• Clear, passport-style photo with solid background required for badge issuance
