Schedule

• Monday to Friday
• 4 days per week, 10-hour shifts (8:00 AM to 6:00 PM)
• Option for 5 days of 8-hour shifts available
• No weekends
• Clinic hours: 7:00 AM to 6:00 PM (hours negotiable)

Job Description

• Fully outpatient physical therapy position
• Preferred experience with orthopedic and neurological physical therapy populations

Required Qualifications

• Valid Physical Therapist license in Kansas
• Basic Life Support (BLS) certification

Required Documentation and Compliance

• Government-issued photo ID (must be carried during work hours)
• Social Security Card or number verification
• Proof of E-Verify or attestation
• Background check covering 7 years of criminal history, education verification, and sex offender registry
• EPLS/GSA/SAM and HHS/OIG checks within 30 days of start and updated with contract renewals
• Education verification of highest degree earned
• Physical exam within the last 12 months
• Negative Quantiferon Gold TB test within 1 year (or T-spot if QGold unavailable), updated annually; if positive, negative chest X-ray within 2 years and annual TB questionnaire required
• Current Tdap vaccine within the last 10 years (or approved exemption)
• Vaccination documentation or declination for MMR, Varicella, Hepatitis B, Influenza, and Tdap prior to start
• Negative 13-panel drug test within 30 days of start (instant tests not accepted)
• References: 2 professional references within 2 years required for travel positions
• Completed employment application/resume

Orientation and Facility Requirements

• Orientation held on-site; business casual attire recommended (scrubs allowed if required)
• Color scrubs allowed for all departments
• Lactation resources available upon request
• Facility-specific forms including vaccination declination, TB questionnaire, respirator clearance, and immunization registry authorization required prior to start

Additional Notes

• Pass/fail competency exams and medication/pharmacology competency testing required as applicable
• Annual updates for skills checklist if applicable
• Must complete orientation checklist to meet facility competency standards
