Outpatient Hospice RN

Location: Albany, Oregon
Employment Type: Contract (13 weeks)
Shift: Varies, includes rotating weekends
Hours per Week: 40

Job Description

Provide hospice nursing care supporting patients and families through the final stages of illness.
Care is delivered in home settings including private residences, skilled nursing facilities, assisted living, and adult foster homes.
Services include intermittent skilled nursing visits, 24-hour on-call nursing, pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained volunteers, and bereavement support.
Medications and equipment are provided as appropriate.

Required Qualifications

Valid RN license in Oregon
Minimum of 1 year of hospice experience, including experience within the last year
BLS certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Valid driver’s license and personal automobile with required insurance; vehicle must be maintained in good working order

Additional Information

Use of personal vehicle required for outpatient hospice visits.
