Description
Physical Therapist – Home Health
Location: Missoula, Montana
Employment Type: Contract
Contract Length: 13 weeks
Schedule: Full-Time, typically Monday through Friday, 8am–5pm, with possible on-call, weekend, and holiday coverage
Hours per Week: 40 hours
Job Summary
Provide evaluation and rehabilitation interventions in patients' homes, including management and supervision of patient care and education for patients and families.
Key Responsibilities
- Evaluate patients and establish individualized plans of care for the home environment.
- Coordinate care with other disciplines and obtain orders for additional services as needed.
- Deliver direct patient care based on care plans and physician orders.
- Re-evaluate patient progress and adjust treatment frequency and duration accordingly.
- Communicate effectively with physicians, staff, and schedulers.
- Document all skilled services provided.
- Maximize consistency of care through effective scheduling.
- Oversee services provided by Physical Therapist Assistants, if applicable.
- Perform other duties as assigned.
Required Qualifications
- Valid Physical Therapist license in Montana
- Basic Life Support (BLS) certification
Additional Information
- Day shift with possible on-call duties.
- Work involves travel to patient homes within the service area.
