Registered Nurse Case Manager - Contract

Location: Glenwood Springs, Colorado
Employment Type: Contract
Contract Length: 4 weeks

Job Description

Full-time day position with weekend rotation, no on-call duties
Schedule: 3 shifts per week, each 12 hours (total 36 hours/week)
Assist inpatients and outpatients and their families with management of their plan of care across the continuum

Required Qualifications

Valid Registered Nurse (RN) license in Colorado
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Minimum 3 years of Care Manager experience, including experience within the last year
Experience with Epic electronic health record system

Additional Information

Potential housing available: newly remodeled furnished studio and one-bedroom apartments near the hospital, including utilities and Wi-Fi (based on availability)
No call responsibilities
Ensure accurate Social Security number and date of birth for onboarding to avoid delays
