Description
Occupational Therapist - Outpatient (Travel/Contract)
Location: Pleasant Prairie, Wisconsin
Contract Length: 13 weeks
Job Details
- Shift:
- 35 hours per week
- Monday, Wednesday, Friday: 7:00 AM – 5:30 PM (10 hours)
- Tuesday: 12:30 PM – 5:30 PM (5 hours)
- Start Date: July 13
- Locals Accepted: Yes (at a lower rate)
- First Time Travelers: Not accepted
Required Qualifications
- Valid Occupational Therapist license in Wisconsin
- Minimum 1 year of Occupational Therapist experience within the last year
- Completion of at least one travel assignment
- Basic Life Support (BLS) certification
Background and Compliance Requirements
- 7-year background check including Social Security trace, county searches for all locations lived/worked/schooled in the last 7 years, nationwide search, and sex offender registry
- Professional references: 2 verified within 2 years of start
- Annual skills checklist completion
Certifications and Competencies
- BLS (Basic Life Support) – ARC or AHA accepted; Provider, Instructor, and RQI acceptable
- Other certifications (ACLS, PALS, NRP, NIH Stroke Certification) may be required depending on specialty but not specified as mandatory for this role
- Medication test/pharmacology competency exam as applicable
- Color blind testing if applicable to specialty (not specifically required here)
Health and Immunization Requirements
- Physical exam within 1 year of start
- Negative QGold TB test within 1 year (TSpot/PPD not accepted) or negative chest X-ray within 60 days of start
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, series, or declination)
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March)
- Negative 10-panel drug screen with expanded opiates within 14 days of start (marijuana positive not disqualifying)
Facility Specific Requirements
- Photo for badge (passport-style, shoulders up, no filters)
- Confidentiality statement signed prior to start
- Compliance checklist completed by agency representative

- Prior travel experience is not required
- This is a travel/contract position with a defined 13-week duration
