Dialysis Registered Nurse - Travel/Contract
Location: Bend, Oregon
Contract Length: 13 weeks
Job Details
- Shift: 4 days per week, 10-hour shifts (8:00 AM – 6:30 PM)
- Patient Ratios: 1:2 during day shifts, 1:1 during night shifts
- On-Call: Required from 6:30 PM to 8:00 AM, plus 24-hour coverage on Sundays
- Weekend Work: Required
- Floating: May float within scope to nearby facilities as needed
- Charge RN Role: May be assigned charge nurse responsibilities
- Holiday Rotation: Included
- Start Date: Must start the week of July 6 (Monday, Tuesday, Wednesday)
- Local Candidates: Not accepted (75-mile radius rule)
- Return to Office Restrictions: 7 days
Required Qualifications
- Valid RN license in Oregon
- Minimum 1 year of dialysis experience within the last year
- ACLS certification
- BLS certification issued by the American Heart Association
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Background and Compliance Requirements
- 7-year criminal background check (including SSN trace, FACIS L3, DOJ sex offender search, education verification) within 90 days of start, updated annually
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Primary source verification of state license
- NURSYS verification required for RN license
- Education verification with diploma or transcript on file
- Facility-specific agreements and confidentiality forms must be signed prior to start
- Passport-style photo required for badge within 48 hours of signing
Health and Safety Requirements
- Physical exam within 1 year of start
- Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (positive titer required; vaccine series alone not sufficient)
- Tetanus/Diphtheria/Tdap vaccine within 10 years or declination accepted
- Negative TB test (QGold) within 1 year; for positive responders, negative chest X-ray within 5 years and annual TB questionnaire required
- Annual TB questionnaire required
- Negative 12-panel drug screen within 90 days of start
- Seasonal flu vaccination or agency declination/exemption required during flu season (ends 3/31)
- COVID vaccine preferred but not required; declination or exemption required if unvaccinated
- Respiratory safety clearance required annually
Competency Requirements
- Medication test/pharmacology competency exam as applicable
- Annual skills checklist and competency exams as applicable
This role is a crisis assignment requiring immediate availability and strong dialysis expertise.
