Home Health RN - Montana, Missoula
Job Type
- Contract
- Full-time (40 hours per week)
- Schedule varies; typical in-home RN schedule is Monday to Friday, 8 AM to 5 PM, with on-call rotation
Job Summary
- Provide skilled nursing, care, and case management for patients in their homes or in an inpatient hospice center
- Admit patients onto services and coordinate care to ensure continuity
- Facilitate quality clinical and cost outcomes through assessment, planning, implementation, and evaluation of care
- Collaborate with an interdisciplinary team to support patient and family needs
- Oversee care provided by Home Health/Hospice Aides and CNAs, ensuring compliance with care plans
- Maintain accurate and complete patient records in accordance with agency standards and regulatory requirements
- Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training
Essential Duties
- Assess and evaluate patients’ physical and psychosocial status, including changes in condition or reactions to treatments/medications
- Provide symptom management and promote optimal health and quality of life through teaching, counseling, and appropriate interventions
- Communicate effectively with other disciplines, community health, support, and social agencies
- Perform home visits and telephone assessments as needed
Required Qualifications
- Valid RN license in Montana
- Minimum 2 years of Home Health experience, including experience within the last year
- Experience in Acute Care
- Experience in Hospice care
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
