Pharmacist (Inpatient) - Travel/Contract
Location: Hood River, Oregon
Contract Length: 13 weeks
Job Summary
Seeking a dedicated outpatient infusion pharmacist with oncology patient care experience. The role involves oversight of medication use in an outpatient infusion center, including order verification, compounding evaluation, treatment plan review, insurance verification, patient education, and management of infusion adverse reactions.
Work Schedule
- Shift: Day (4 shifts of 10 hours each)
- Hours per week: 40
- Block schedules required
- No floating between units
- No on-call or weekend requirements
- Patient ratio approximately 1:20

Required Qualifications
- Valid Pharmacist license in Oregon
- Minimum 1 year of pharmacist experience within the last year
- Experience in an outpatient setting
Certifications
- BLS (Basic Life Support) - AHA or Canadian Heart & Stroke Foundation accepted
- ACLS (Advanced Cardiac Life Support) - AHA or Canadian Heart & Stroke Foundation accepted
- Chemotherapy certification (if required, verify with clinical track)
Background and Compliance Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable)
- Background check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
- Education verification required
- Employment application covering at least last 5 years
- Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
- Drug screening (9 panel) within 12 months of start
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health screenings:
- TDAP within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months, updated annually
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
- COVID vaccination record preferred (declinations not required)
- Respiratory safety clearance required annually for travel candidates
Additional Requirements
- Passport-style badge photo with neutral background
- Completion of annual regulatory and specialty-specific training modules via LMS
- Competency exams as applicable to profession and specialty
- Medication/Pharmacology competency exam required
- No prior travel experience explicitly required

- No 48-hour shifts approved
- No billable orientation hours (0 hours NBO vs. standard 40)
- Must verify certifications and requirements with clinical track or unit director as needed
