Physical Therapist - Home Health (Travel/Contract)

Location: Dover, Delaware
Contract Length: 13 weeks

Job Description

• Provide physical therapy services in a home health setting with home visits and periodic office meetings.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
• Must be willing to travel up to 75 miles from the home clinic. Mileage reimbursed at IRS standard rate (patient home to patient home).
• Use of EMR system: HCHB.
• Productivity expectation: Minimum of 30 points per week (points assigned based on visits and activities).
• Weekend and on-call requirements vary by location; discuss during interview.
• Local candidates accepted at the same rate.

Required Qualifications

• Minimum 1 full year of recent adult home health physical therapy experience (first-timers not accepted).
• Current Physical Therapist license in hand (pending licenses accepted only if license can be obtained within a few days).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver’s license required.
• Agency-required liability insurance required.

Additional Requirements & Compliance

• Must complete a standard 10-panel drug screen.
• Physical exam within 12 months prior to start, confirming good health and no communicable diseases; updated annually.
• Negative TB test or chest X-ray within 12 months, updated annually; results cannot be pending at start.
• Immunization or titer proof required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccines, or history of chickenpox)
• Seasonal flu vaccination or documented declination required.
• COVID-19 complete vaccination series or declination required.
• Must provide two professional references.
• Work history verification for at least two most recent employers or last 7 years.

Background Checks & State-Specific Requirements

• Comprehensive background checks covering the last 7 years of residence, work, and education.
• Sex offender registry checks and nationwide criminal searches.
• State-specific background and fingerprinting requirements must be met prior to start (varies by state).
• Various authorizations and releases required depending on state placement.

Orientation & Compliance Training

• Must review and sign multiple compliance documents prior to start, including but not limited to:

  • Code of Conduct
  • HIPAA and Privacy Policy
  • Compliance Training Certification
  • Confidentiality Forms
  • Home Health Core Policy and Scenarios
  • COVID-19 Policy and Attestation
• Annual completion of supplier packets required if working more than one year consecutively or after any break in service.

Travel & Work Conditions

• Contractor will be assigned 5-10 patients per day, equating to roughly 6-8 points daily.
• No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.
• Schedule typically standard 5x8 (Monday-Friday), though some positions may have alternate shift schedules (e.g., 4x10 Friday-Monday).
• No weekend rotation or on-call required for this assignment.
