Case Manager - Hospice

Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Shift: Monday to Friday, 8:00 AM – 5:00 PM
Hours per Week: 40

Job Description

Provide compassionate hospice nursing care, supporting patients and families with strength and courage.
Serve as a primary nurse with an assigned caseload, leading the interdisciplinary team for those patients.
Deliver care in various settings including private homes, nursing homes, group homes, and assisted living facilities.
Act as a teacher to patients and caregivers, helping identify personal goals of care and building caregiver confidence.
Manage symptom control and perform comprehensive assessments requiring critical thinking.
Enjoy flexibility in daily scheduling to support work/life balance.
Occasional weekend and on-call backup required.

Required Qualifications

Valid Registered Nurse (RN) license in Colorado
Minimum of 2 years of Hospice Palliative Care RN experience, including experience within the last year
Basic Life Support (BLS) certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
