Physical Therapist - Home Health (Travel/Contract)

Location: West Plains, Missouri
Contract Length: 13 weeks

Job Details

• Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
• Weekends Required: No
• Call Required: No
• Floating Required: No
• Caseload: 8 to 11 patients daily depending on travel time
• Productivity Expectation: 4 to 5 patients per day depending on area and travel
• EMR System: Meditech
• Other Therapists on Staff: 1
• Mileage Reimbursement: IRS Standard Mileage Rate in effect at time of travel

Required Qualifications

• Valid Physical Therapist license in Missouri
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification

Additional Requirements

• Government-issued photo ID must be carried during hours of service
• Proof of E-Verify or attestation required
• Background checks including a 7-year criminal history, SS trace, counties nationwide, sex offender registry, and facility-level checks must be completed and updated annually
• Federal exclusions screenings (EPLS/GSA/SAM, HHS/OIG) required within 30 days of start and updated for contract renewals
• Physical exam attesting ability to meet physical demands, updated annually
• Negative TB test or chest X-ray within 1 year, updated annually
• Immunizations or titers required: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella
• Drug screening: 9-panel drug test within last 12 months (instant acceptable)
• Seasonal flu vaccination or declination required annually (October 1 – March 31)
• COVID-19 seasonal vaccine required or approved medical/religious exemption
• Proof of 2 clinical references or hospital evaluations
• Completion of employment application/resume and skills checklist updated annually
• Facility-specific policies include a nicotine-free campus and dress code adherence

• First-time travelers are accepted for this position
• Proposed schedule may be negotiable to Tuesday through Friday if needed
