Occupational Therapist - Home Health (Travel/Contract)
Location: Augusta, Georgia
Employment Type: Travel/Contract
Shift: Day (5x8 hours), 8:00 AM – 4:30 PM
Hours per Week: 40
Job Summary
- Provide occupational therapy services in a home health setting focused on geriatric patients.
- Average daily caseload: approximately 6 visits, with a full-time productivity expectation of around 25 units per week.
- Floating within facility/scope of practice as needed.
- No weekend shifts required.
- Must be available to work major holidays if they fall within the assignment period (expectation to work 1-2 major holidays depending on number during contract).
- Mileage reimbursement provided for travel within multiple counties; average daily travel 50-100 miles.
Contract Length
- 12 weeks
Required Qualifications
- Valid Occupational Therapist license in Georgia.
- Minimum 1 year of experience as an Occupational Therapist, with experience within the last year.
- Basic Life Support (BLS) certification (AHA or ARC accepted).
- Experience with Homecare Homebase (HCHB) required until 10/1/2026; Epic experience required after 10/1/2026.
- If no prior HCHB experience, must complete an online HCHB course before starting (cost paid upfront by clinician).
Compliance & Onboarding
- Full compliance with credentialing and onboarding required to start on time; delays may result in a 2-week pushback due to orientation scheduling.
- Background check required within 30 days prior to start, including education verification and criminal history.
- Photo ID and clear passport-style photo required for badge.
- Must review and sign facility dress code and code of conduct policies prior to start.
- Proof of current automobile insurance required if using personal vehicle for travel.
Health & Safety Requirements
- Physical exam within 12 months of start date required.
- Negative TB test (PPD, QGold, or TSpot) within 12 months or proof of prior positive with negative chest x-ray within 6 months. TB questionnaire required annually.
- Immunizations or proof of immunity required:
- Measles (Rubeola), Mumps, Rubella (2 MMR vaccines or positive titer)
- Varicella (2 vaccines or positive titer)
- Hepatitis B surface antibody titer or signed declination
- Seasonal flu vaccine or signed declination (required during flu season)
- Negative 10-panel drug screen within 60 days of start (including amphetamines, benzodiazepines, cocaine, opiates, methadone, PCP, THC, barbiturates, propoxyphene).
- Tdap vaccine highly recommended but not required.
Additional Requirements
- Skills checklist updated within the last 12 months.
- Verification of most recent work history required; can be pending at start with proof it is in process.
- Two clinical references required.
- Must pass color blindness test or alternative assessment if applicable.
- Competency exams required by agency (not client) for medication/pharmacology and general competencies.
Work Environment & Other Details
- Scrub color: any.
- Return to Office (RTO) maximum 7 days.
- No call requirements specified.
- Locals accepted at same rate.
