Job Opportunity: Home Health Registered Nurse

Location: Portsmouth, New Hampshire
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
  • Average patient load: 5-8 patients per day.
  • Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
  • Willingness to travel up to 75 miles from the home clinic; approximately 250 miles traveled weekly.
  • Charting required using HomeCare HomeBase (HCHB) and OASIS (for RNs).
  • Typical schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have a 4x10 schedule (Friday-Monday).
  • No guaranteed hours; patient census may vary.

Required Qualifications

  • License: Current, valid Registered Nurse (RN) license for New Hampshire (license must be in hand; no pending licenses accepted).
  • Experience: Minimum 1 full year of recent Adult Home Health or Hospice nursing experience (depending on specialty). First-time travelers are not accepted; recent specialty experience is mandatory.
  • Certifications: Current CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
  • Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; if not current, retraining is required.
  • RNs must be proficient in OASIS documentation.

Work Environment & Expectations

  • Dress code: Scrubs.
  • No weekend rotation or on-call required (may vary; discuss during interview).
  • Responsible for documentation, travel between patient homes, and patient care activities.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not reimbursed).
  • Local candidates accepted within the same pay rate.

Compliance & Documentation Requirements

  • Must complete a comprehensive background check including:
    • 7-year residential/work history verification.
    • State-specific criminal background checks and sex offender registry searches.
    • Fingerprinting as required by state regulations.
  • Required health documents:
    • Physical exam within 12 months confirming good health and no communicable diseases.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
    • Drug screening (10-panel).
    • Seasonal flu vaccination or documented declination.
    • COVID-19 vaccination series or declination.
  • Must have a valid driver’s license and current personal or auto insurance covering the candidate as a driver.
  • Must review and sign various compliance, confidentiality, privacy, and policy documents prior to start and annually if working consecutively over one year.

Additional Notes

  • Candidates must be willing to complete required quizzes and policy acknowledgments related to home health or hospice care prior to assignment start.
  • References: Two professional references required.
  • Work history verification for the last 7 years or at least two most recent employers is mandatory.

This position is ideal for experienced Registered Nurses with recent home health or hospice experience who are comfortable working independently in a community setting and managing a patient caseload with travel requirements.
