Description
Job Opportunity: Physical Therapist - Home Health
Location
- Hackensack, New Jersey
- Must be willing to travel up to 75 miles from home clinic, covering approximately 200-500 miles per week within Bergen County, NJ.
Employment Type
- Travel/Contract
- Typical contract length: 13 weeks
Schedule
- Monday to Friday, 8:00 AM to 5:00 PM (5x8 hour shifts)
- Weekend rotation: None
- On-call: None
Patient Load & Productivity
- Average patients per day: 5-6
- Weekly productivity expectation: 30 points (points assigned based on visit type and activities)
- Types of cases: Start of Care, Follow Ups
Required Qualifications
- Minimum 1 full year of recent Adult Home Health experience (first-time home health therapists not accepted)
- Current Physical Therapist license in hand (pending licenses accepted only if a few days from issuance)
- CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted)
- Reliable transportation and current driver's license
- Agency-required liability insurance
- Experience with Homecare Homebase EMR preferred
Additional Requirements
- Must complete various background checks including 7-year history, fingerprinting (state-specific), and drug screening (10-panel)
- Required health documents: Physical exam within 12 months, negative TB or chest X-ray within 12 months, immunizations or titers for MMR, Hepatitis B, Varicella, and seasonal flu vaccination or declination
- Must review and sign confidentiality, privacy, compliance, and other agency-specific policies prior to start
- Two professional references required
- Verification of at least two most recent employers or last 7 years of work history
Travel & Logistics
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed)
- Local candidates accepted at the same rate
Notes
- Schedule and on-call requirements may vary by location; discuss during interview
- Some positions may have alternate shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8s Monday-Friday
This position is ideal for experienced Physical Therapists with recent adult home health experience who are comfortable traveling within a defined territory and managing a moderate patient load in a home health setting.
