OT Home Health - Portland, OR

Job Details

  • Specialty: Therapy/Rehabilitation (Occupational Therapy)
  • Location: Portland, Oregon (and surrounding areas)
  • Employment Type: Travel/Contract
  • Contract Length: 13 weeks
  • Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
  • Hours per Week: 40
  • Work Setting: Home Health
  • Patient Ratio: 1:1, may visit up to 6 patients per day (30 productivity points standard)
  • Schedule Notes: No weekend requirement; facility cannot accommodate 4x10 or 3x12 shifts; no on-call or floating between units; no block scheduling
  • Local Candidates: Accepted within 50-75 mile radius, considered local

Required Qualifications

  • Valid Occupational Therapist license in Oregon
  • Minimum 1 year of Occupational Therapist experience within the last year
  • BLS (Basic Life Support) certification
  • Experience in a Home Health setting
  • Experience with OASIS documentation

Facility & Work Environment

  • Attire: Business casual; scrubs allowed but not required; no jeans
  • Charting: EPIC Remote Client (laptop) and Rover app (phone) for documentation
  • Parking: Free parking available
  • Mileage Reimbursement: At IRS standard rates

Compliance & Background Requirements

  • Background Check: Must have a background check within 90 days covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide sex offender, FACIS Level 3, education verification, motor vehicle record (required for home health), and state-specific abuse/neglect searches as applicable
  • Education Verification: Required for all candidates; must verify related degree or highest education level
  • Drug Screen: 9-panel drug screen within 12 months of start; instant tests acceptable
  • References: Minimum 2 references covering 1 year of employment within last 3 years in comparable specialty/setting
  • Employment Application: Must include at least last 5 years of employment with month/year format; 7 years required for advanced practice nurses or subcontractors
  • Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO; must indicate free of infectious disease and ability to work with or without accommodation
  • Immunizations & Health Requirements:
    • TDAP vaccine within 10 years (declination accepted)
    • Negative TB test or chest x-ray within 12 months (annual updates required)
    • MMR (Measles, Mumps, Rubella): 2 vaccines or positive titer; declination accepted
    • Varicella: 2 vaccines or positive titer; declination accepted
    • Hepatitis B: Documentation of vaccination series or positive titer; declination accepted
    • Seasonal Flu Vaccine during flu season (9/1–3/30); declination accepted
  • Respiratory Safety Clearance: Required annually for travel candidates; virtual questionnaire reviewed by licensed healthcare professional
  • Fit Mask Testing: Required annually for travel candidates; onsite testing available if no valid record within 1 year

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful.
