Description

Occupational Therapist - Acute Care Hospital

Location: Pleasant Prairie, Wisconsin
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Details

  • Start Dates: 7/18, 7/27, 8/10
  • Shift: Day shift, 4 days per week, 10 hours per day (7:30 AM - 6:00 PM)
  • Work Days: Weekends required
  • Setting: Hospital acute care

Required Qualifications

  • Valid Occupational Therapist license in Wisconsin
  • Minimum 1 year of Occupational Therapist experience, including experience within the last year

Certifications & Competencies (as applicable)

  • Basic Life Support (BLS) - ARC or AHA accepted
  • Advanced Cardiac Life Support (ACLS) - ARC or AHA accepted (not required for CNAs)
  • Pediatric Advanced Life Support (PALS) - ARC or AHA accepted (if applicable)
  • Neonatal Resuscitation Program (NRP) - as required
  • Medication/Pharmacology competency exam (as applicable)
  • Competency exams relevant to profession/specialty

Background & Screening Requirements

  • 7-year background check including SS trace, county searches for all locations lived/worked/schooled, nationwide sex offender check (must be within 1 year of start and updated annually)
  • Wisconsin Caregiver Search Results within 90 days of start (agency to run search)

Health & Immunization Requirements

  • Physical exam within 1 year of start
  • Negative QGold TB test within 1 year of start (TSpot/PPD not accepted) or negative chest X-ray within 60 days of start
  • Immunity or vaccination proof for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Varicella
    • Hepatitis B (titer, series, or declination)
  • Negative 10-panel drug screen with expanded opiates within 14 days of start (instant tests not accepted; marijuana positive not disqualifying)
  • Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March)

Documentation & Compliance

  • Photo for badge (passport-style, shoulders up, no filters)
  • Confidentiality statement signed prior to start
  • Compliance checklist to be completed by agency representative
  • References: 2 professional references verified within 2 years of start
    • For inpatient RNs only: 1 reference must be from most recent acute care assignment with performance comments (not applicable to OT)
  • Annual skills checklist per agency standards

Additional Notes

  • Flu vaccine religious or medical exemption forms must be completed if applicable
  • Dosimeter request form required only for imaging department (not applicable to OT)

This position offers an opportunity to work in an acute care hospital setting with a defined schedule and clear credentialing and compliance requirements. Prior acute care experience and licensure in Wisconsin are essential.

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. Cons

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
