Description
Physical Therapist - Acute Care Hospital (Travel/Contract)
Location: Pleasant Prairie, Wisconsin
Contract Length: 13 weeks
Shift: Weekend Days, 4 shifts of 10 hours each (7:30 AM - 6:00 PM)
Hours per Week: 40
Job Description
- Work in an acute care hospital setting.
- Weekend shifts only.
- Locals accepted, but at a lower rate.
- First-time travelers accepted.
Required Qualifications
- Valid Physical Therapist license in Wisconsin.
- Minimum 1 year of Physical Therapist experience within the last year.
Certifications & Competencies (as applicable)
- BLS (Basic Life Support) – ARC or AHA accepted.
- ACLS (Advanced Cardiac Life Support) – ARC or AHA accepted (not required for CNAs).
- PALS (Pediatric Advanced Life Support) – ARC or AHA accepted.
- NRP (Neonatal Resuscitation Procedure) – as required.
- NIH Stroke Certification – required for ICU, IMC/IMCU, inpatient, Med Surg, Step Down RN positions (may not apply to PT but check facility requirements).
- Medication Test/Pharmacology Competency Exam – required as applicable.
- Color Blind Testing – required for dialysis/kidney center staff only.
- Competency Exam – required as applicable to profession/specialty.
Background & Screening Requirements
- 7-year background check within 1 year of start, including SS trace, county searches for all locations lived/worked/schooled, nationwide search, and sex offender registry.
- Wisconsin Caregiver Search Results required within 90 days of start (agency to run).
Health & Immunization Requirements
- Physical exam within 1 year of start.
- Negative QGold TB test within 1 year of start (TSpot/PPD not accepted). Negative chest X-ray within 60 days if applicable.
- Immunizations or positive titers required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, series, or declination)
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March).
- Negative 10-panel drug screen with expanded opiates within 14 days before start (instant tests not accepted). Positive marijuana test is not disqualifying.
Documentation & Other Requirements
- Photo for badge (passport-style, shoulders up, no filters).
- Confidentiality statement signed prior to start.
- Compliance checklist to be completed by agency representative.
- References: 2 professional references verified within 2 years of start. For inpatient RNs only, one must be from most recent acute care assignment (not applicable to PT).
- Annual skills checklist as per agency standards.
Additional Notes
- Prior travel experience is not required.
- Locals accepted at a lower rate.
- Start dates available: July 18, July 27, August 10.
