Description
Registered Nurse - Case Management (Travel)
Location: Houston, Texas
Contract Length: 13 weeks
Job Summary
- Coordinate management of care to ensure optimal resource utilization, service delivery, and compliance with external review agencies.
- Provide comprehensive assessment, care planning, implementation, and evaluation of individual patient needs.
- Enhance patient management quality and satisfaction, promoting continuity of care and cost-effectiveness.
- Integrate case management, utilization review, and discharge planning functions.
- Serve as a patient advocate addressing holistic patient needs through resource utilization and discharge planning.
Work Schedule
- Monday to Friday, 8:00 AM to 4:30 PM
- Approximately 36 hours per week
Required Qualifications
- Valid RN license in Texas
- Minimum 5 years of Acute Care Case Management experience, including experience within the last year
- Experience in a Long Term Acute Care (LTAC) setting
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- BLS certification issued by the American Heart Association
- Strong interdisciplinary team (IDT) collaboration skills
