Description
PT Home Health (Travel/Contract)
Location: Portland, Oregon (also covers Clark County, Washington and surrounding areas)
Contract Length: 13 weeks
Job Details
- Specialty: Therapy/Rehabilitation (Physical Therapy)
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Work Setting: Home Health
- Patient Load: 1:1 ratio, up to 6 patients per day (30 productivity points standard)
- Start Date: ASAP
- Schedule Notes:
- No weekend work required
- No on-call duties
- No floating between units
- No block scheduling or extended shifts (no 4x10s or 3x12s)
- Local Candidates: Accepted if within 50-75 mile radius, considered local
Required Qualifications
- Valid Physical Therapist license in Oregon
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Experience with OASIS documentation
- Basic Life Support (BLS) certification issued by the American Heart Association or Heart and Stroke Foundation
Facility & Work Environment
- Attire: Business casual preferred; scrubs allowed but no specific color required; no jeans
- Charting: EPIC Remote Client (laptop), Rover app (phone), and OASIS documentation
- Parking: Free
- Mileage Reimbursement: IRS standard rates
Additional Notes
- Facility may call off up to one shift per pay period (every two weeks)
- Confirm return-to-office (RTO) restrictions with hiring manager
- Annual regulatory and specialty-specific training modules are generally not required for Home Health at this facility
- Candidates must complete state-specific background and licensure attestations as applicable for Oregon and Washington
- Float policy acknowledgment may be required, but no floating is expected for this role
