Description

Physical Therapist - Home Health (Travel/Contract)

Location: Roseburg, Oregon
Contract Length: 13 weeks


Job Overview

  • Provide physical therapy services in a home health setting with home visits and periodic office meetings.
  • Average patient load: 5-6 patients per day, with 5-10 patients assigned daily equating to roughly 6-8 points per day (30 points expected weekly).
  • Types of cases: Start of Care and Follow Ups.
  • Schedule: Primarily Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Some positions may have a 4x10 schedule (Friday-Monday). Weekend and part-time options may be available; discuss during interview.
  • Must be willing to travel up to 75 miles from the home clinic, averaging about 100 miles per week. Mileage reimbursed at IRS standard rate (patient-to-patient travel compensated; commute to/from first and last patient is not billable).
  • Use of EMR systems PointCare and HCHB is required.
  • Local candidates accepted at the same rate.

Required Qualifications

  • Valid Physical Therapist license in Oregon
  • Physical Therapist (1 year of experience)
  • Physical Therapist experience within the last year
  • One completed travel assignment
  • BLS issued by American Heart Association or American Red Cross
  • Experience in a Home Health setting
  • Driver's License
  • CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
  • Reliable transportation and a current driver’s license required.
  • Agency-required liability insurance must be maintained.

Additional Requirements

  • On-call and weekend requirements vary by location; discuss during interview.
  • Must complete and maintain all required background checks, drug screening, and health documentation including:
    • Standard 10-panel drug screen.
    • Physical exam within 12 months confirming good health and no communicable diseases.
    • Negative TB test or chest X-ray within 12 months.
    • Immunization or titer proof for Measles, Mumps, Rubella, Hepatitis B, Varicella.
    • COVID-19 vaccination series or documented declination.
    • Seasonal flu vaccination or declination documentation.
  • Must complete all required facility-specific compliance documents and training prior to start, including confidentiality, privacy policies, HIPAA, compliance training, and home health core policy acknowledgements.
  • Two professional references required.
  • Verification of at least two most recent employers or last 7 years of work history.

Travel Experience

  • Prior travel experience is not required for this position.

Work Environment

  • Home health setting with travel to patient homes.
  • No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.

Summary

This is a travel contract opportunity for a Physical Therapist with at least one year of recent adult home health experience. The role involves providing therapy in patients’ homes within a 75-mile radius of Roseburg, Oregon, with a typical 40-hour workweek and some flexibility for weekend or part-time schedules. Candidates must hold a current PT license, CPR/BLS certification, and meet all health and background screening requirements. Reliable transportation and liability insurance are mandatory.
