Physical Therapist - Home Health (Travel/Contract)

Location: West Plains, Missouri
Contract Length: 13 weeks

Job Details

• Employment Type: Travel/Contract
• Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
• Weekends, call, and floating are not required
• Caseload: 8 to 11 patients daily depending on travel time
• Productivity expectation: 4 to 5 patients per day depending on area and travel
• Electronic Medical Record (EMR) system: Meditech
• Number of other therapists on staff at this location: 1

Required Qualifications

• Valid Physical Therapist license in Missouri
• Minimum 1 year of Physical Therapist experience, including experience within the last year
• Basic Life Support (BLS) certification

Compliance and Documentation Requirements

• Government-issued photo ID must be carried during hours of service
• Proof of E-Verify or attestation required
• Background checks: 7-year criminal check within a year of start, updated annually; includes SS trace, counties, nationwide, sex offender, and facility-level checks
• Federal exclusions screening (EPLS/GSA/SAM and HHS/OIG) required within 30 days of start and updated for contract renewals
• Physical exam attesting ability to meet physical demands, updated annually
• Negative TB test or chest X-ray within 1 year, updated annually
• Immunizations or titers required: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella
• Drug screening: 9-panel drug test within last 12 months (instant acceptable)
• Seasonal flu vaccination or declination required annually (October 1 – March 31)
• COVID-19 seasonal vaccine required or approved medical/religious exemption
• Proof of 2 clinical references or hospital evaluations
• Completion of employment application/resume and skills checklist updated annually
• Facility is a nicotine-free campus

Additional Notes

• Mileage reimbursement at IRS standard rate applies
• Proposed schedule may be negotiable (e.g., Tuesday to Friday)
• Orientation includes pass/fail testing (EKG/medication) and competency exams as applicable
• Advanced certifications such as ACLS, PALS, NRP, and TNCC may be required as applicable but not specified as mandatory for this role

This position offers an opportunity for Physical Therapists with at least one year of experience to work in a home health setting on a travel contract basis in Missouri.
