Visit Nurse, Home Health
Overview
- Location: Port Angeles, Washington
- Schedule: Full-time, 40 hours/week, 5x8s
- Shift: Day shift, weekends/holidays
- Contract length: 13 weeks
Role Summary
Provides nursing visits in patients’ homes as part of the interdisciplinary care team, supporting continuity of care and coordinating with the case manager RN.
Required Qualifications
- Associate’s degree
- At least 1 year of recent experience in home health, acute care, or long-term care
- Current Washington State Registered Nurse license
- Current BLS certification
- Valid Washington driver’s license
- Proof of insurance for the automobile used for work
- Reliable automobile transportation to travel to patient residences
- Driving record that meets current employer standards
- Cell phone with activated voicemail
- Computer internet connectivity
