Job Overview

Role Summary

• Acts as a liaison for patients, families, and the multidisciplinary clinical team.
• Assesses patient needs across clinical, emotional, spiritual, psychosocial, financial, and related areas.
• Coordinates discharge planning and helps ensure discharge instructions are understood and followed.
• Educates patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms.
• Coordinates multidisciplinary consultations and tumor conferences, including documentation of recommendations.
• Supports development of treatment plans, patient education materials, clinical pathways, and quality studies.
• Serves as a clinical partner and escalation point for Intake Coordinators.
• Promotes community education, prevention awareness, and early-detection screening programs.

Required Qualifications

• Valid RN license in Indiana.
• Oncology experience within the last year.
• 1 year of oncology experience.
• Associate of Science in Nursing / Associate Degree in Nursing.
• BLS certification issued by the American Heart Association.
• Driver’s license or state ID.
• 2 references.
• Current resume covering the last 7 years of employment.
• Work history verification covering the last 7 years of employment.
• Skills checklist for the traveler’s unit and any units they may float to.

Additional Requirements

• Background check.
• Education verification.
• NURSYS verification.
• Fingerprinting is required for positions that do not require an Illinois state license.
• Drug screen.
• Physical exam within 1 year and updated annually.
• Fit mask within 1 year of start.
• Tetanus/Diphtheria/Tdap documentation.
• Negative TB test or chest X-ray.
• Measles, mumps, rubella, hepatitis B, and varicella documentation.
• Flu vaccination/documentation or approved exemption.
• Facility orientation modules before clearance/start.
• Photo for badge.
• COVID-19 religious or medical exemption form if applicable.
