Job Overview

Employment type: Travel
Location: Dover, New Jersey
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Hours per week: 40
Contract length: 13 weeks

Role Summary

Coordinates patient care across the continuum through collaboration with the interdisciplinary care transitions team.
Supports assessment, care planning, plan implementation, evaluation, utilization review, discharge planning, and continuity of care.

Required Qualifications

Active New Jersey RN license or multi-state RN license
BLS certification through the American Heart Association
4 years of recent RN case management experience
Experience in an LTAC or STACH setting
Experience with MCG
Experience with discharge planning across acute care settings
Strong interdisciplinary team collaboration experience

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. She helped me with onboarding and getting used to the timesheet app and she answers my texts promptly, even after hours. She is a great recruiter liason. Gladys was a super helpful with onboarding and Denise will hook you up with benefits with no trouble at all. Great team to work with.

Cons

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
