- This facility cannot accommodate 4x10s or 3x12s

Required Qualifications
- Valid RN license in Oregon
- Home Health (1 year of experience)
- Home Health experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or Canadian Heart & Stroke Foundation
- Associate of Science in Nursing / Associate Degree in Nursing
- Experience in a Home Health setting
- Experience with OASIS
Key Responsibilities
- Home health visits in the West Salem/Keizer area and surrounding areas, with potential coverage for Woodburn
- May visit up to 6 patients per day
- Expected to have personal transportation
- Occasional travel radius may exceed 50 miles
Additional Notes
- First-time travelers are not accepted
- Local candidates are accepted within a 50-mile radius
- Primary series COVID vaccination plus one booster is required
- Epic experience is preferred
- Guaranteed hours may be reduced by up to 1 shift per pay period
- Free parking is available

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. She helped me with onboarding and getting used to the timesheet app and she answers my texts promptly, even after hours. She is a great recruiter liason. Gladys was a super helpful with onboarding and Denise will hook you up with benefits with no trouble at all. Great team to work with.

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
