Description

Physical Therapist — Home Health

Job Overview

  • Contract role in home health
  • Location: Albuquerque, New Mexico
  • Schedule: Monday–Friday
  • Hours: 30 per week
  • Contract length: 13 weeks
  • Service area includes a 45-mile radius around Albuquerque

Required Qualifications

  • Valid Physical Therapist license in New Mexico
  • BLS
  • Driver's License

Experience

  • Two years of clinical experience preferred
  • Home health experience is a plus, but not required

Key Responsibilities

  • Provide physical therapy services within the legal scope of practice
  • Complete evaluations, progress notes, reassessments, recertifications, and discharge-related documentation
  • Develop, implement, and update individualized plans of care
  • Deliver skilled interventions and maintenance therapy programs
  • Supervise Physical Therapist Assistants and Home Health Aides as allowed by state practice acts
  • Consult with physicians and interdisciplinary team members on treatment and patient progress
  • Educate patients and caregivers on home exercise, mobility, and adaptive strategies
  • Complete OASIS assessments and related documentation
  • Apply ICD-10 coding accurately
  • Participate in quality improvement, staff education, interdisciplinary meetings, case reviews, and continuing education
  • Follow infection control, safety, risk prevention, HIPAA, privacy, and confidentiality standards
  • Work independently and communicate professionally with patients, families, and team members

Additional Notes

  • Basic computer proficiency is needed, with willingness to learn EMR systems and OASIS documentation
  • The role requires use of organizational technology systems and tools for communication, documentation, productivity, and data management

Marigold is polite and professional. She goes above and beyond to ensure the contractor is well informed and has everything needed to be successful. She helped me with onboarding and getting used to the timesheet app and she answers my texts promptly, even after hours. She is a great recruiter liason. Gladys was a super helpful with onboarding and Denise will hook you up with benefits with no trouble at all. Great team to work with.

Cons

Onboarding is somewhat tedious, but they do dot every "i" and cross every T.
