Physical Therapist — Home Health
Job Overview
- Contract role in home health
- Location: Albuquerque, New Mexico
- Schedule: Monday–Friday
- Hours: 30 per week
- Contract length: 13 weeks
- Service area includes a 45-mile radius around Albuquerque
Required Qualifications
- Valid Physical Therapist license in New Mexico
- BLS
- Driver's License
Experience
- Two years of clinical experience preferred
- Home health experience is a plus, but not required
Key Responsibilities
- Provide physical therapy services within the legal scope of practice
- Complete evaluations, progress notes, reassessments, recertifications, and discharge-related documentation
- Develop, implement, and update individualized plans of care
- Deliver skilled interventions and maintenance therapy programs
- Supervise Physical Therapist Assistants and Home Health Aides as allowed by state practice acts
- Consult with physicians and interdisciplinary team members on treatment and patient progress
- Educate patients and caregivers on home exercise, mobility, and adaptive strategies
- Complete OASIS assessments and related documentation
- Apply ICD-10 coding accurately
- Participate in quality improvement, staff education, interdisciplinary meetings, case reviews, and continuing education
- Follow infection control, safety, risk prevention, HIPAA, privacy, and confidentiality standards
- Work independently and communicate professionally with patients, families, and team members
Additional Notes
- Basic computer proficiency is needed, with willingness to learn EMR systems and OASIS documentation
- The role requires use of organizational technology systems and tools for communication, documentation, productivity, and data management
