Job Overview

Required Qualifications
- Valid RN license in Illinois
- CVICU experience with 2 years of experience
- CVICU experience within the last year
- ACLS issued by the American Heart Association
- BLS issued by the American Heart Association or the American Red Cross
- NIHSS
- Associate of Science in Nursing / Associate Degree in Nursing
Required Certifications
- BLS
- ACLS
- NIH Stroke Certification
Required Pre-Start Items
- Government-issued photo ID
- 2 forms of ID
- Emergency contact form
- Employment application
- Work history verification for the last 3 years
- At least 2 recent references
- Physical exam
- N95 fit test
- Tdap vaccination or signed declination
- Negative TB screening or chest X-ray
- Measles, mumps, rubella, hepatitis B, and varicella immunity documentation or acceptable alternatives
- 10-panel drug test
- Flu vaccination or declination during flu season
- Background check and required screening verifications
- Required onboarding forms, acknowledgements, and training modules before start
Additional Notes
- Color blindness testing is required
- Annual renewal is required for several compliance items and competencies
